those under five years old. Vitamin D deficiency is not only an independent risk factor for the occurrence of pneumonia but is also associated with more severe cases of pneumonia. This study aims to investigate the relationship between children’s vitamin D status and the severity of pneumonia in hospitalized cases. An analytical observational study using a cross-sectional design was conducted from December 2023 to June 2024 at Dr. Mohammad Hoesin Hospital, Palembang. The study involved 100 pediatric patients aged 1 month to 18 years with pneumonia (n = 50) and severe pneumonia (n = 50) requiring intensive care in the PICU, followed by blood sampling to measure 25-hydroxyvitamin D (25(OH)D) levels. Data were analyzed using chi-square and Mann-Whitney tests, with statistical significance set at p < 0.05. The 25(OH)D levels in children with severe pneumonia were 21.65 (3.5 – 56.10) ng/mL, and in children with pneumonia were 19.75 (3.2 – 44) ng/mL (p=0.885). No significant association was found between vitamin D deficiency status and pneumonia severity (OR=1.62; 95% CI 0.73-3.58; p=0.229). Age, gender, nutritional status, socioeconomic status, immunization status, exclusive breastfeeding, smoke exposure, and illness duration > 3 days were not significantly associated with severe pneumonia (p>0.05). However, prior treatment history, comorbidities, and fever > 38°C were protective factors against severe pneumonia in this study (p < 0.05). These findings indicate that the association between lower vitamin D levels and pneumonia severity in children aged 1 month to 18 years remains unclear, suggesting the need for further research with narrower age groups and consideration of factors affecting vitamin D levels.
