Abstract
ABSTRACT Predictor Model Clinical Score for Congenital Heart Disease Background: Congenital heart disease (CHD) is a structural heart defect present from birth that requires timely diagnosis, especially in regions with limited access to pediatric cardiology specialists and echocardiography. This study aimed to develop a clinical scoring system to screen for CHD effectively in children by identifying key clinical signs. Methods: This diagnostic study employed a cross-sectional approach at RSUP Mohammad Hoesin Palembang from January to June 2024, involving children under 18 years suspected of CHD. Clinical signs analyzed included murmur, shortness of breath, feeding difficulties, and recurrent respiratory infections. Logistic regression determined the scoring weight for each sign, forming the basis of a clinical score for CHD screening. Results: A predictor model clinical scoring system was established with scores of 3 for murmur, 2 for shortness of breath, 1 for feeding difficulties, and 1 for recurrent respiratory infections. Using a cutoff score of ≥4, the clinical scoring system for congenital heart disease achieves a specificity of 94.33% and a sensitivity of 58.29%. Conclusion: This predictor model clinical score offers a valuable screening tool for CHD in settings with limited access to advanced diagnostics, facilitating prompt referral to pediatric cardiologists. Further research is recommended to validate the reliability of this scoring system across diverse healthcare settings.
