Abstract

Background: Diabetic Retinopathy (DR) is a microvascular complication of Diabetes Mellitus (DM) that can lead to visual impairment and blindness. The severity of DR is often correlated with decreased visual acuity, but other factors such as age, gender, intraocular pressure, glycemic control, hypertension, and the presence of macular edema may also contribute. This study aims to analyze the relationship between the severity of diabetic retinopathy and visual acuity, as well as exploring other factors affecting visual acuity in type II DM patients at RSUP Dr. Mohammad Hoesin Palembang. Methods: This cross-sectional study used medical records of type II DM patients with DR at RSUP Dr. Mohammad Hoesin Palembang from January 2021 to October 2024 that met the inclusion and exclusion criteria. Data were analyzed using bivariate and multivariate tests to determine the relationship between DR severity and visual acuity. Results: From 62 patients (124 eyes), most were aged < 60 years (71,0%) and female (61,3%). A total of 58,9% of eyes were classified as Proliferative Diabetic Retinopathy (PDR), and 36,3% experienced blindness (visual acuity < 3/60). Fisher’s exact test showed a significant relationship between DR severity and visual acuity (p = 0,008). Multivariate analysis revealed that DR severity had the strongest association with visual acuity (p = 0,021), while age, gender, intraocular pressure, HbA1c, hypertension, and macular edema were not significant. Conclusion: DR severity is significantly associated with visual acuity, while other factors were not statistically significant statistically.
