Abstract
Background: Epilepsy is a neurological disorder characterized by a recurrent tendency to experience epileptic seizures, which impact neurobiological, cognitive, psychological and social aspects. The use of anti-epileptic drugs (OAEs) is known to cause long-term side effects, one of which is metabolic syndrome. The prevalence of metabolic syndrome among epilepsy patients ranges from 23.5% to 52.6%. Methods: This study was an analytical study of cross-sectional design, using secondary data from the medical records of epilepsy patients at the Outpatient Clinic of RS Mohammad Hoesin Palembang from March to May 2023. Data were analyzed using univariate and bivariate with the Chi Square test to evaluate the relationship between the number, mechanism, and duration of AED use, as well as confounding variables (age, gender, and occupation), with metabolic syndrome. Results: Among 76 patients studied, 25% had metabolic syndrome. Age, gender, occupation and number of OAEs were not significantly associated with metabolic syndrome. However, OAE mechanism (p = 0.035) and duration of OAE use (p = 0.017) showed significant association. Patients using enzymatic-mechanism OAEs with duration > 3 years had a seven times higher risk than those using OAEs ≤ 3 years (OR 7.00; IK95% 1.45-33.69, p value = 0.020) of developing metabolic syndrome. Conclusion: The mechanism and duration of OAE use increase the risk of metabolic syndrome in patients with epilepsy. Long-term clinical monitoring is needed to prevent metabolic complications due to OAE therapy.
