Abstract
Background: Fluid management is one of the main pillars in the management of sepsis and sepsis shock, where proper fluid management can avoid the most frequent complication of sepsis, namely acute kidney injury. Methods: A retrospective cohort study was conducted on 295 adult sepsis patients admitted to the ICU between January 2022 and December 2024. Fluid overload was categorized using the cumulative fluid balance ROC curve. Statistical analysis included Chi-Square, T-test, and multivariate logistic regression to analyze the association of fluid overload with AKI. Results: Fluid overload was significantly associated with AKI (p<0.001) with an RR value of 12.878 (95% CI = 6.952-23.857). The ROC curve showed excellent predictive value of cumulative fluid balance on AKI. The cut off point value of cumulative fluid balance was >-77 ml. Multivariate analysis found fluid overload as an independent risk factor. Discussion: The results of this study support evidence from previous studies that fluid overload is significantly associated with AKI. The provision of fluid therapy is a management that must be done wisely and carefully. Fluid administration in critical patients consists of four stages known as the ROSE concept, namely resuscitation, optimization, stabilization, and evacuation. The evacuation phase includes the process of deresuscitation which has an important role in patients who experience fluid overload. Conclusion: Fluid overload is an excellent predictor of AKI. Individualized and restrictive fluid strategies have been shown to reduce the risk of AKI in sepsis patients in the ICU. Keywords: Sepsis, Fluid Overload, Acute Kidney Injury
