Abstract
Background: Endometriosis is a chronic disease characterized by the presence of endometrial tissue outside the endometrial cavity. The immunologic system is known to play a role in the pathogenesis of endometriosis including pro-inflammatory cytokine IL-6, and anti-inflammatory cytokine IL-10. With current treatment methods both operative and hormonal therapy including Dienogest, the recurrence rate of endometriosis remains high, so it is necessary to look for other therapeutic modalities. Bone Marrow Mesenchymal Stem Cell (BMMSC) is a multipotent stem cell that is immunomodulatory so it is expected to be useful as a therapy for endometriosis. Objective: This study aims to prove the effectiveness of BMMSC administration on suppressing the inflammatory process by reducing IL-6 expression and increasing IL-10 expression in endometriosis model mice. Methods: This study is a Randomized Controlled Trial in vivo with post test only control group design. The research sample consisted of 32 mice that met the inclusion criteria and then induced endometriosis for 14 days. The samples were divided into 4 groups: negative control group (K), BMMSC only (P1), dienogest only (P2), and a combination of BMMSC + dienogest (P3), the groups were treated for 14 days. On day 29, immunohistochemical examination was carried out to assess the expression of IL-6 and IL-10 in the peritoneal tissue of model mice. Results: The results of One-Way Anova test showed there was a significant difference in IL-6 expression between groups (p<0.001), with the lowest expression rate in the BMMSC + dienogest combination group. In IL-10 expression, there was also a significant difference between groups (p<0.001), with the highest expression rate in the BMMSC + dienogest group. In the posthoc test, it was found that the BMMSC + Dienogest combination group most significantly reduced IL-6 expression compared to the control group (MD 2.98; p<0.001). The combination of BMMSC and Dienogest most significantly increased IL-10 expression compared to the control (MD -4.10; p<0.001). Conclusion: BMMSC and Dienogest contributed to a decrease in IL-6 and an increase in IL-10 expression, with a greater synergistic effect in the combination than the monotherapy.
