Abstract

Background: Chronic kidney disease (CKD) is a progressive kidney disorder that results in high healthcare costs. Renal fibrosis involves complex pathophysiological mechanisms, including apoptosis (programmed cell death). One of the key parameters regulating apoptosis is the anti-apoptotic protein B-cell lymphoma-2 (BCL-2). Progressive and unresolved renal fibrosis can lead to permanent kidney dysfunction, necessitating Renal Replacement Therapy (RRT). Therefore, more effective treatment strategies are needed, including the use of natural compounds such as Cinnamomum burmanii (CB), which has potential antifibrotic effects. This study aims to determine the effect of administering the bioactive fraction of CB DLBS 2411 on changes in BCL-2 levels in a rat model of kidney fibrosis. Methods: This study was a laboratory-based experimental study with a pre-post test with a control group design. A total of 30 male Sprague-Dawley rats were subjected to Unilateral Ureteral Obstruction (UUO) and divided into five groups: a negative control group, a positive control group receiving methylprednisolone, and three treatment groups receiving CB DLBS 2411 bioactive fraction at doses of 50 mg/kgBW, 100 mg/kgBW, and 200 mg/kgBW. Urea and creatinine levels were measured as standard parameters, and BCL-2 levels were assessed before and after UUO, as well as after treatment. Data were analyzed using the Statistical Package for the Social Sciences (SPSS). Results: The rat model of kidney fibrosis with UUO was successfully established, as evidenced by increased levels of urea, creatinine, and BCL-2. Administration of the bioactive fraction DLBS2411 from Cinnamomum burmannii resulted in a decrease in BCL-2 levels, although the difference was not statistically significant compared to the negative control. Additionally, DLBS2411 demonstrated effectiveness comparable to methylprednisolone in reducing creatinine and urea levels, particularly at a dose of 200 mg/kg BW. Conclusion: The bioactive fraction of CB DLBS 2411 exhibited a similar effect to methylprednisolone in reducing BCL-2 levels, suggesting its potential as an adjunctive therapy for kidney fibrosis and as an alternative to methylprednisolone therapy.
