Abstract

Latar belakang: Insiden epilepsi semakin meningkat dan kontrol bangkitan merupakan fokus utama penatalaksanaan. Quantitative Electroencephalography (QEEG) merupakan metode analisis lanjutan EEG yang dapat mengevaluasi abnormalitas gelombang dasar otak. Pada pasien epilepsi, abnormalitas QEEG seperti peningkatan gelombang lambat (delta dan teta) sering dihubungkan dengan kontrol bangkitan yang buruk. Tujuan: Mengevaluasi hubungan abnormalitas QEEG dengan kontrol bangkitan pada pasien epilepsi.di RS Mohammad Hoesin Palembang Metode: Penelitian ini merupakan studi analitik potong lintang terhadap 185 pasien epilepsi dewasa yang menjalani pemeriksaan EEG pada April–September 2024. Data QEEG dikaji berdasarkan parameter relative power, absolute power, Delta-Alpha Ratio (DAR), dan Peak Alpha Frequency (PAF). Hubungan dengan kontrol bangkitan dianalisis menggunakan uji chi-square dan regresi logistik multivariat. Hasil: Mayoritas pasien tergolong epilepsi tidak terkontrol (71,9%) dan memiliki lesi struktural (57,8%). Relative power delta tinggi (OR = 3,30; p = 0,009), teta tinggi (OR = 3,78; p < 0,001), dan beta tinggi (OR = 0,31; p = 0,007) memiliki hubungan signifikan dengan epilepsi tidak terkontrol. Sementara itu, tidak terdapat hubungan bermakna pada karakteristik klinis, absolute power, DAR, dan PAF. Regresi logistik multivariat menunjukkan bahwa delta tinggi (OR = 3,12) dan teta tinggi (OR = 3,69) memiliki hubungan signifikan dengan epilepsi tidak terkontrol. Kesimpulan: Abnormalitas pada relative power gelombang delta dan teta tinggi berhubungan signifikan dengan epilepsi tidak terkontrol, menunjukkan potensi QEEG sebagai alat bantu dalam evaluasi dan penatalaksanaan epilepsi. Kata kunci: Epilepsi, QEEG, relative power, kontrol bangkitan Background: The incidence of epilepsy has shown a gradual increase , and seizure control remains the main focus of management. Quantitative Electroencephalography (QEEG) is an advanced EEG analysis method used to evaluate abnormalities in baseline brain wave activity. In epilepsy patients, QEEG abnormalities such as increased slow-wave activity (delta and theta) are frequently associated with poor seizure control. Objective: To evaluate the relationship between QEEG abnormalities and seizure control in patients with epilepsy. Methods: This was a cross-sectional analytical study involving 185 adult epilepsy patients who underwent EEG examination at Mohammad Hoesin Hospital Palembang between April and September 2024. QEEG data were analyzed based on relative power, absolute power, Delta-Alpha Ratio (DAR), and Peak Alpha Frequency (PAF). Associations with seizure control were assessed using chi-square tests and multivariate logistic regression. Results: The majority of patients had uncontrolled epilepsy (71.9%) and structural brain lesions (57.8%). High relative power in delta (OR = 3.30; p = 0.009), theta (OR = 3.78; p < 0.001), and high beta waves (OR = 0.31; p = 0.007) were significantly associated with uncontrolled epilepsy. No significant associations were found for clinical characteristics, absolute power, DAR, or PAF. Multivariate logistic regression showed that high delta (OR = 3.12) and high theta (OR = 3.69) remained significantly associated with uncontrolled epilepsy. Conclusion: Abnormalities in high relative power of delta and theta waves are significantly associated with uncontrolled epilepsy, highlighting the potential role of QEEG as a supportive tool in the evaluation and management of epilepsy.
