Abstract

Background: Peptic ulcer perforation is a surgical emergency requiring prompt intervention. Risk stratification using prognostic scoring systems can help optimize patient management. This study aims to evaluate the predictive performance of the Mannheim Peritonitis Index and Boey Score in estimating mortality risk in patients with gastric perforation. Methods: A retrospective study was conducted on 31 patients who underwent laparotomy for gastric perforation at RS Mohammad Hoesin Palembang between January 2023 and December 2024. Results: The optimal cutoff for Mannheim Peritonitis Index (≥22) demonstrated a sensitivity of 87.5% but with a lower specificity of 40%. Compared to the Boey Score, Mannheim Peritonitis Index had an accuracy of 74.19%, with a sensitivity of 89.47% and a specificity of 50.00%. Discussion: This study supports previous findings that both the Boey Score and Mannheim Peritonitis Index can predict mortality risk. Mannheim Peritonitis Index can be a useful tool for assessing the prognosis of patients with gastric perforation, particularly in identifying those at high risk of mortality. However, due to its lower specificity, the use of Mannheim Peritonitis Index should be considered alongside other clinical factors to avoid excessive therapeutic decisions. Conclusion: Both Boey Score and Mannheim Peritonitis Index demonstrate moderate predictive performance in estimating mortality risk in gastric perforation. While Mannheim Peritonitis Index is advantageous in identifying high-risk patients, Boey Score remains useful for early decision making. Further research with larger cohorts is recommended to refine risk stratification tools and improve clinical outcomes.
