Abstract

Latar Belakang: Karsinoma ovarium merupakan salah satu penyebab utama keganasan ginekologi dengan angka mortalitas tinggi. Berdasarkan mutasi, karsinoma ovarium dibagi menjadi tipe I dan tipe II. Karsinoma ovarium tipe II lebih agresif dan sering terdiagnosis pada stadium lanjut, dengan kadar CA-125 yang jauh lebih tinggi dibandingkan tipe I. Karsinoma ovarium tipe I (±30% kasus) dianggap berkembang lebih lambat dan lebih sering ditemukan pada stadium awal. Karsinoma ovarium tipe II (±70% kasus) memiliki prognosis buruk, Studi menunjukkan bahwa PD-L1 berperan dalam mekanisme immune evasion, memungkinkan sel kanker menghindari pengenalan oleh sistem imun dan meningkatkan kelangsungan hidupnya dalam lingkungan mikro tumor yang imunosupresif. Ekspresi PD-L1 yang tinggi dikaitkan dengan resistensi terhadap kemoterapi, terutama pada pasien dengan mutasi BRCA1/2. Penelitian ini bertujuan untuk mengetahui hubungan ekspresi PD-L1 dengan pendekatan klinis seperti Usia, Stadium FIGO dan serum CA125. Metode: Penelitian deskriptif analitik cross-sectional menggunakan arsip karsinoma ovarium yang telah didiagnosa secara histopatologi selama periode 1 Januari 2021-30 Desember 2024, sebanyak 32 sampel dipilih dengan non-random sampling dan dibagi menjadi karsinoma ovarium tipe I dan tipe II. Data klinikopatologi didapat dari rekam medis. Imunohistokimia PD-L1 dilakukan untuk menentukan ekspresi PD-L1. Dilakukan analisis bivariat untuk melihat apakah terdapat hubungan ekspresi PDL-1 dengan karakteristik klinikopatologi karsinoma ovarium. Hasil: Didapatkan mayoritas 53,1% pasien berusia ≥55 tahun, terbanyak pada stadium FIGO III (65,6%), karsinoma ovarium terbanyak yaitu tipe I (53,1 %) dan nilai ekspresi PDL-1 rendah sebanyak 43,8%. Berdasarkan analisis bivariat tidak terdapat hubungan bermakna antara ekspresi PD-L1 dengan usia(p=0.494), stadium FIGO (p=0.086). dan dengan karsinoma ovarium (p = 0.626). Kesimpulan: Tidak terdapat hubungan yang bermakna antara ekspresi PD-L1 dengan usia, stadium FIGO, karsinoma tipe I dan tipe II. Kata Kunci: Karsinoma ovarium, PD-L1, tipe mutasi, tipe I dan tipe II
