Abstract
Background: Impetigo is a superficial epidermal infection most often caused by Staphylococcus aureus and/or Streptococcus pyogenes. Fusidic acid is one of the first-line topical therapies for impetigo in Indonesia. Antibiotic resistance is a challenge in treatment. Pineapple core extract (Ananas comosus (L.) Merr.) has major bioactive content such as bromelain which is antibacterial. Objective: Comparing the susceptibility of Staphylococcus aureus and Streptococcus pyogenes to pineapple core extract and fusidic acid in impetigo patients. . Method: Experimental in vitro research with post-test only control group design. The research samples were all Staphylococcus aureus and Streptococcus pyogenes bacteria from impetigo patients at the DVE Polyclinic, RSMH Palembang for the period January-March 2025. Resistance testing was performed with the disc diffusion method utilizing pineapple core extract (50%, 70%, and 100%) alongside fusidic acid. Results: The Mean diameter of the inhibition zone against the growth of Staphylococcus aureus by pineapple core extract at concentrations of 50%, 70%, and 100% were 6.74 ± 0.462 mm, 7.48 ± 0.522 mm, and 8.51 ± 0.578 mm, respectively. The mean diameter of the inhibition zone against Streptococcus pyogenes by pineapple core extract at concentrations of 50%, 70%, and 100% were 8.58 ± 0.349 mm, 9.59 ± 0.532 mm, and 10.69 ± 0.540 mm, respectively. Bivariate analysis showed a significant difference in the diameter of the inhibition zone of Staphylococcus aureus between the 10 µg fusidic acid group and 3 groups of pineapple core extract (p < 0.05), while in Streptococcus pyogenes there was no significant difference in the diameter of the inhibition zone of Streptococcus pyogenes between the 10 µg fusidic acid group and 100% pineapple core extract (p> 0.05). Multivariate analysis showed that there were significant differences between five groups (p = 0.000; p < 0.050). The susceptibility test of Staphylococcus aureus to fusidic acid was found to be 63.3% sensitive. The susceptibility of Streptococcus pyogenes to fusidic acid showed 100% sensitive. Conclusion: Pineapple core extract 100% has inhibition zone comparable to fusidic acid against Streptococcus pyogenes in impetigo, but the inhibition zone of pineapple core extract against Staphylococcus aureus is still lower than fusidic acid.
