Abstract
Background: Type 2 Diabetes Mellitus (T2DM) is a global health problem with increasing prevalence. Individuals with a family history of T2DM, especially first-degree relatives, have a higher risk of developing the disease. Chronic inflammation and immune dysregulation are believed to play a role in T2DM pathogenesis. The Systemic Immune-Inflammation Index (SII) and High-sensitivity C-reactive Protein (hs-CRP) are systemic inflammatory markers that have been associated with various metabolic conditions. This study aimed to compare SII values and hs-CRP levels between individuals with normal glycemic status and prediabetes who have a first-degree family history of T2DM. Methods: This analytical observational study with a cross-sectional approach involved 60 respondents, grouped into normal (n=30) and prediabetes (n=30) groups based on HbA1c levels. Platelet count, neutrophil count, lymphocyte count, Systemic Immune-Inflammation Index (SII), and High-Sensitivity C-Reactive Protein (hs-CRP) levels were measured and compared between the two groups. Independent T-test and Mann-Whitney U test were used for statistical analysis. Results: In individuals with a family history of type 2 diabetes, the prediabetes group (n=30) compared to the normal group (n=30) showed higher HbA1c levels (5.9±0.2% vs 5.3±0.3%), higher platelet count (prediabetes: 375.7±57.9 x 10^9/L vs normal: 289.1±66.8 x 10^9/L), higher neutrophil count (4.5±1.0 x 10^9/L vs 3.4±0.9 x 10^9/L), higher SII values (641.5±126.9 vs 375.5±68.9), and higher hs-CRP levels (2.5±1.2 mg/L vs 0.5±0.3 mg/L), all statistically significant (p<0.05). There was no difference in lymphocyte count (prediabetes: 2.6±0.5 x 10^9/L vs normal: 2.6±0.8 x 10^9/L) between the two groups. Conclusion: There were significantly higher Systemic Immune-Inflammation Index (SII) and High-Sensitivity C-Reactive Protein (hs-CRP) levels in the prediabetes group.
