Abstract

Background: Type 2 diabetes mellitus (T2DM) is characterized by hyperglycemia due to impaired insulin secretion and/or insulin resistance, with low-grade chronic inflammation playing a key role in its pathogenesis. Diacerein has anti-inflammatory effects and improves insulin sensitivity; however, clinical evidence in the Indonesian population remains limited. This study aimed to evaluate the effect of diacerein addition on serum hsCRP levels and glycemic control in uncontrolled T2DM patients Methods: This double-blind randomized controlled trial was conducted on uncontrolled T2DM patients at Mohammad Hoesin General Hospital (RSMH) Palembang, who received standard therapy plus either diacerein 50 mg twice daily or placebo for 90 days. Measured parameters included hsCRP, glycated hemoglobin (HbA1c), and fasting blood glucose (FBG) before and after intervention. Bivariate and multivariate analyses were used to assess the intervention effect and baseline factors influencing changes. Results: Diacerein administration resulted in a significantly reduced HbA1c (–1.08 ± 1.15%) and FBG (–66.47 ± 76.37 mg/dL) compared with placebo (p=0.002 and p=0.018, respectively). No significant changes in hsCRP were observed in either the diacerein or placebo groups, and the between-group difference was not statistically significant (p = 0.084). Multivariate analysis identified diacerein as an independent predictor of HbA1c reduction (B = –1.082; p = 0.020) and FBG reduction (B = –30.295; p = 0.019). Higher baseline HbA1c was associated with greater reductions in HbA1c, whereas higher baseline hsCRP was linked to smaller decreases in HbA1c. Meanwhile, FPG reduction was greater in patients with higher baseline HbA1c but smaller with higher baseline FPG. No serious adverse events were observed; mild diarrhea occurred in one patient and resolved spontaneously. Conclusion: The addition of diacerein in patients with uncontrolled type 2 diabetes mellitus significantly reduced HbA1c and fasting plasma glucose (FPG) levels but did not produce a meaningful change in hsCRP levels. HbA1c reduction was greater in patients with higher baseline HbA1c but smaller in those with elevated baseline hsCRP. FPG reduction was also greater in patients with higher baseline HbA1c but smaller in those with higher baseline FPG.
