Abstract
Background: Hearing loss associated with chronic suppurative otitis media (CSOM) predominantly results from ossicular damage, although it is relatively less frequent in CSOM without cholesteatoma. Recognizing of possible ossicular damage prior to surgery is essential for effective surgical planning. This study aims to identify factors that can be used to predict ossicular damage in CSOM without cholesteatoma. Methods: An analytical observational cross-sectional study was conducted at Dr. Mohammad Hoesin General Hospital from May to June 2025. A total of 157 medical records of patients with CSOM without cholesteatoma who underwent surgical procedures between January 2021 and December 2024 met the inclusion and exclusion criteria. Results: Ossicular damage was found in 26.8% of patients. Bivariate analysis identified the following predictive factors for ossicular damage in CSOM without cholesteatoma: gender (OR = 2.694; p = 0.013), age (OR = 3.084; p = 0.012), middle ear mucosa condition (OR = 51.451; p < 0.001), and air-bone gap with a cutoff point of 37.25 dB (OR = 3.107; p = 0.012). Logistic regression analysis revealed that the predictor factors for ossicular damage in CSOM without cholesteatoma were middle ear mucosa (Adjusted OR: 52.795; p < 0.001), age (Adjusted OR: 3.610; p = 0.020), gender (Adjusted OR: 3.226; p = 0.011), and air-bone gap (Adjusted OR: 2.660; p = 0.032). Conclusion: The predictive factors for ossicular damage in CSOM without cholesteatoma are middle ear mucosa condition, age, gender, and air-bone gap.
