Abstract

Neutrophil to Lymphocyte Ratio (NLR) has been used to reflect disease severity in Myasthenia Gravis. This study aimed to investigate the correlation between NLR and MGFA score. This cross-sectional study was conducted from July 2024 to July 2025. 50 Myasthenia Gravis patients aged ≥18 years were included using total sampling. Data on NLR and MGFA scores were collected from medical records and analysed using Spearman’s rank correlation with a significance level of p<0.05. Ethical approval was obtained from the Ethics Committee. Most Myasthenia Gravis patients were female (64%) and aged <45 years (60%). Elevated NLR (≥2) was found in 78% of patients, with a mean of 13.58 ± 20.7. The majority were classified as MGFA I (40%) and MGFA III (26%). There was a significant positive correlation between NLR and MGFA scores (r = 0.617; p < 0.001). NLR showed good diagnostic performance (AUC = 0.854; 95% CI = 0.743–0.965; p = 0.003) with an optimal cut-off > 6.6, sensitivity 100% and accuracy 72%. NLR showed a significant positive correlation with MGFA scores, indicating that higher NLR values reflect greater disease severity in Myasthenia Gravis. These findings suggest that NLR may serve as a simple and accessible biomarker for assessing disease progression, although further prospective studies with larger samples are required to validate its diagnostic utility.
