Abstract
Introduction: Prevalence of autistic spectrum disorder (ASD) is rising. Nowadays, ASD only diagnosed by Diagnostic Manual of Mental Disorder-5 (DSM-5) anamnesis-based criteria. Mechanism underlying ASD is not clear, it is believed immune dysregulation that cause lymphocyte monocyte ratio (LMR) decreases. Recent study suggests that there is negative correlation between LMR and ASD. The aim of study is find the RLM validity in diagnosing ASD. Methods: This diagnostic test use secondary data from ASD-suspected children medical record in RSMH by time period January–November 2019. Data that fulfil inclusion and exclusion criterias analyzed by data processor software to determine cut-off point and diagnostic value. Results: There are 69 subjects, 26 ASD and 43 non-ASD. Most of ASD subjects are aged 18 months–<5 years (20/26), male (21/26), have normal birth weight (23/26), spontaneously delivered (16/26), second child and so on (14/26), have good nutritional status (20/26) and all of subjects have complete basic immunization history (26/26). LMR at cut-off point 8.29 is not significant in diagnosing ASD (p=0.27, p>α) which has very weak diagnosis power (AUC=0.58), sensitivity 80.77%, specificity 44.19%, positive predictive value 46.67%, negative predictive value 20.83%, positive likelihood ratio 1.45, negative likelihood 0.44, prevalence 0.37, and accuracy 57.80%. Conclusion: Lymphocyte monocyte ratio is very weak in diagnosing autistic spectrum disorder. Keywords: lymphocyte monocyte ratio, autistic spectrum disorder
