Abstract

Primary dentists have their function as a gatekeeper who give the first primary health care. The payment system for the primary dentist is 2.000 IDR each patient by using capitation method. The primary dentist should use the fund properly. The aims of this study to analyze the allocation, utilization, and transparency of capitation funds for primary dentists. This research was conducted qualitatively with in-depth interviews and observations. The informants were selected with purposive sampling methods which consist of 16 (sixteen) primary dentist both as independent and clinical practitioners located in Palembang and Lubuklinggau cities with highest and lowest capitation coverage. Data were analyzed by using Miles and Huberman methods including data reduction, presentation, conclusion and verification. The results showed that the dental capitation funds that received by dentist in Palembang and Lubuklinggau still vary based on the patient registered. In Palembang, the highest capitation funds was found in the pratama clinic dentist with income of IDR 24,741,120 and the lowest capitation funds was found in independent practice dentists with income of IDR 158,100. In Lubuklinggau City, the highest and lowest capitation funds were found in independent practice dentists with income of IDR 16,058,000 and IDR 681,700. The primary dentist with high capitation fund allocate service cost more than operational cost. The primary dentist with lowest capitation fund allocate operational cost more than service cost. The pattern of allocation and utilization of capitation funds for the high capitation primary dentist is sufficient, but not for the primary dentis with low capitation, especially for the service cost. It can be concluded that the distribution of the registered patient at the primary dentist both in Palembang and Linggau are not balanced. The capitation funds are not balanced with the operational cost that should be paid by the dentist. Most of the operational cost is used for medical consumables product. The suggestion of this study is BPJS Kesehatan should review again dental capitation fund, the distribution of the patient registered and dental regulation for the National Health Insurance.
