Abstract
Metastasis otak merupakan jenis keganasan sistem saraf pusat dengan tingkat mortalitas yang tinggi dan angka kejadian yang dapat mencapai 3-10 kali melebihi jumlah tumor otak primer. Kanker paru merupakan sumber metastasis utama bagi metastasis otak maupun metastasis medula spinalis. Penelitian ini merupakan penelitian deskriptif cross-sectional retrospektif dengan menggunakan data rekam medik. Pengambilan sampel dilakukan secara total sampling dengan total 41 subjek. Analisis data menggunakan uji univariat menggunakan program SPSS. Penelitian ini dilakukan di RSUP Dr. Mohammad Hoesin Palembang pada bulan Oktober – November 2021. Pada penelitian terhadap 41 subjek, ditemukan rerata usia penderita 54 tahun, kelompok usia terbanyak 51 – 60 tahun dan 61 – 70 tahun (34,1%), proporsi terbanyak merupakan laki-laki (73,2%), pendidikan terakhir SLTA (68,3%), pekerjaan karyawan swasta dan buruh, tani, serta nelayan (26,8%), 26,8% dengan riwayat merokok, 14,6% dengan riwayat penyakit paru dan ekstraparu, dan semua pasien tidak memiliki riwayat keluarga. 90,2% dengan tipe kanker paru NSCLC dengan batuk sebagai gejala klinis yang paling sering ditemukan (56,1%), 82,9% pasien kanker paru ditegakkan pada stadium lanjut, 14,6% pasien juga mengalami metastasis tulang, dan 48,8% pasien diperiksa dengan pemeriksaan histopatologi dan radiologi, 31,7% pasien kanker paru diberikan kemoterapi sebagai tatalaksana spesifik. 75,6% pasien menderita metastasis otak dan 19,5% menderita metastasis medula spinalis. Nyeri dan defisit nervus kranialis merupakan gejala klinis metastasis yang paling sering ditemukan (68,3%). Lesi metastasis otak mayoritas bermanifestasi sebagai lesi multipel (70,7%), dengan waktu awitan synchronous (46,3%). Pasien paling sering diperiksa dengan MRI (39%) dan diberikan radioterapi sebagai tatalaksana spesifik (24,4%). Kata kunci: kanker paru, metastasis otak, metastasis medula spinalis Brain metastases is a malignancy of the central nervous system with a high mortality rate and an incidence that can reach 3-10 times more than primary brain tumors. Lung cancer is the main source of metastases for brain and spinal cord. This study is a retrospective cross-sectional descriptive study using medical record data. This research was conducted at RSUP Dr. Mohammad Hoesin Palembang in October – November 2021. In this study was found that the average age of subjects were 54 years old, and subjects mostly came from 51-60 years and 61-70 years (34.1%) age group, the highest sex proportion was male (73 .2%), the last education was high school (68.3%), 26.8% patients was found with a history of smoking, 14.6% with a history of pulmonary and extrapulmonary diseases, and all patients had no family history. 90.2% had NSCLC lung cancer type with cough as the most common clinical symptom (56.1%), 82.9% of lung cancer patients diagnosed at an advanced stage, 14.6% of patients also had bone metastases, and 48, 8% of patients were examined by histopathological and radiological examination, 31.7% of lung cancer patients were given chemotherapy as a specific treatment. 75.6% of patients had brain metastases and 19.5% had spinal cord metastases. Pain and cranial nerve deficits were the most common clinical symptoms of metastases (68.3%). The majority of brain metastatic lesions manifested as multiple lesions (70.7%), with synchronous time of onset (46.3%). Patients were most frequently examined by MRI (39%) and given radiotherapy as a specific treatment (24.4%). Keywords: lung cancer, brain metastases, spinal cord metastases
