Abstract
Introduction: Diabetes mellitus is a chronic metabolic disease characterized by increased levels of blood glucose (or blood sugar), the level of sugar in the skin (skin glucose) under normal circumstances is 55% of blood glucose levels.In diabetes the ratio increases to 69-71% due to increased blood sugar levels.Increased skin glucose levels is what facilitates the onset of various manifestations of abnormalities in the skin, this condition is called skin diabetes.This study aims to determine the distribution of the frequency of diabetic patients with skin disorders based on risk factors, laboratory examinations, and find out the effect on the incidence of skin diseases in patients with diabetes mellitus at Dr. Mohammad Hoesin Palembang Hospital for the period 2018-2020. Methods: This study is an analytical descriptive study with observational studies. The study will be conducted from September-December 2021 and uses secondary data on diabetes mellitus patients with manifestations of skin disorders in 2018-2020.The data will be processed and analyzed using univariant and bivariant analysis. Result: It found that patients with skin disorders associated with diabetes mellitus there were 76 samples (90,48%), the most manifestations of skin diseases were diabetic ulcers occurred in 62 samples (73,81%) then patients with fungal skin infections was seen in 8 samples (9,52%), bacterial infections occurred in 4 samples (4,76%) and patients with diabetic bullae occurred in 2 samples (2,38%). Patients who experience other skin disorders that often occur in patients with diabetes mellitus obtained as many as 8 samples (9,52%) with samples experiencing pruritus as many as 2 samples (2,38%) and experiencing scabies infections as much as 2 samples (2,38%) and patients with vasculysis, lichen simplex chronicus, xerosis cutis, and erythroderma each was seen in 1 sample (1,19%). From 84 diabetes mellitus patients who experienced manifestations of skin disease were in the age range of 61-80 years (66,66%), women (64,28%), and diabetes mellitus patient with blood sugar levels HbA1c above 7% most experienced manifestations of skin diseases as much as (75%). The results of bivariate analysis were conducted on risk factors for age, sex, and blood sugar levels HbA1c against the type of manifestation of skin diseases that occur most, namely diabetic ulcers.There were significant results in the risk of age to the incidence rate of diabetic ulcers (p-value 0,006), while for sex and blood sugar levels HbA1c did not get significant results with p-values of 0,258 and 0,190. Conclusion: Based on this study, women with diabetes mellitus patients experienced more skin diseases with an age range of 61-80 years and found in patients with HbA1c levels >7%. The most common skin manifestations were diabetic ulcers (73,81%) and had a meaningful association with age risk factors, while in gender and HbA1c levels there was no meaningful association.
