Abstract
Background: Malaria is one of the infectious disease that most often kills people in the world. Plasmodium are parasites that responsible for the increasing number of malaria, including P.falciparum, P.vivcoc, P.malariae and P.ovale. However, among this four species, P.falciparum can cause cerebral complications that would be bad for patients. WHO in 2006 reported about the prevalence (of malaria case in the risk area is 7,48%. In it is development, chloroquineas the first line drug for malaria treatment has run into resistance. Resistance to chloroquinedue to the gene mutation of P falciparum from Pfmdrl codon 86 resulting in over expressed of pghl protein. This leads to increase thechloroquine efflux from food vacuole of P falciparum to cytoplasm so that the concentration of chloroquine inside food vacuole decreased and the effect of chloroquine is not optimal. This study aimed to identify the gene mutation of Pfmdrl codon 86 as the resistance marker of Plasmodium falciparum to chloroquine in patients with malaria in South Sumatera. Method: This study is a research lab and observational descriptive study of 30 malaria falciparum patients. Identification of gene polymorphisms Pfmdrl 86N/Y performed by PCR amplification and RFLP using enzyme AfIII. Result: Genotype distribution of N, N/Y and Y genes Pfmdrl 86N/Y on the subject of this study are each 0 (0%), 0 (0%), and 20 (100%). In the present study, there were no homozygous wild-type genotype (N) and heterozygous genotype (N/Y), while the most common genotype is homozygous mutant genotype (Y). Conclusion: All the isolates was carrying a point mutation N86Y Pfmdrl gene with the most dominant type genotype is homozygous mutant genotype. It is provide that chloroquine is no longer adequate to treat malaria in South Sumatera.
