This management study aims to determine the risk factors that can affect HbA1c levels in patients with Type 2 Diabetes Mellitus in participants of Program Pengelolaan Penyakit Kronis (PROLANIS) at Puskesmas Sekip in 2021. This study is an analytic observational study with a cross-sectional design. The data was taken from a printed-out questionnaire using a google form, then processed using the SPSS. In a total of the 70 research subjects, PROLANIS participants who had the highest controlled HbA1c levels at age >45 years were 19 (27.1%), female gender was 12 (17.1%), waist circumference with central obesity group was 18 (25,7%), duration of disease >3 years as many as 19 (27.1%), moderate comorbidities as many as 13 (18.6%), standard combination drug therapy as many as 15 (21.4%), eating patterns according to the Piringku model T as many as 17 (24.3%), low adherence to taking medication as much as 14 (20%), and light and moderate physical activity each as much as 9 (12.9%).
