Abstract

Paranoid schizophrenia is the most common type of schizophrenia, characterized by difficulty distinguishing between reality and thoughts, which impacts cognitive processes. Patients with this condition have particular vulnerability to medication side effects, especially anticholinergics that can affect cognitive function. This study aimed to analyze the effect of anticholinergic use on cognitive function impairment in outpatients with paranoid schizophrenia at Ernaldi Bahar Hospital, South Sumatra Province. An observational study with a cross-sectional design was conducted on 37 paranoid schizophrenia outpatients during November-December 2024. Data collection included medical record review and interviews, with cognitive function assessment using the Six-item Cognitive Impairment Tests (6 CIT). Statistical analysis was performed using the Chi-square test. The distribution of anticholinergic use showed level 0 at 22 drugs (22.22%), level 1 at 23 drugs (23.24%), level 2 at 9 drugs (9.09%), and level 3 at 45 drugs (45.45%). Cognitive function evaluation in patients using anticholinergics revealed 2 patients (5.40%) with normal cognitive function, 5 patients (13.51%) with mild cognitive impairment, and 30 patients (81.09%) with severe cognitive impairment. Statistical analysis demonstrated a significant effect of anticholinergic use on cognitive function impairment (p = 0.004).
