Abstract

Stunting is a global health issue that affects children's physical and cognitive growth, including in Indonesia, with Banyuasin Regency as one of the affected areas. This study aimed to analyze the implications of maternal health literacy, breastfeeding practices, and growth monitoring on stunting incidence. A case-control design was used with 214 infants (107 stunted, 107 normal) selected through cluster sampling. Data were collected using questionnaires and anthropometric measurements and analyzed using Chi-Square tests, logistic regression, path analysis, and Multi-Group Analysis (MGA). Maternal health literacy related to breastfeeding was significantly associated with stunting (p=0.002; OR=2.884) and influenced the duration of breastfeeding (t=4.683; p=0.000; coef=0.252), which in turn affected infant nutritional status (t=3.306; p=0.001; coef=0.209). However, literacy regarding growth monitoring showed no significant association with monitoring behavior or nutritional status. Exclusive breastfeeding was the dominant factor associated with stunting (p<0.001; OR=9.700), with non-exclusively breastfed infants having a 9.7 times higher risk of stunting. Growth monitoring also showed a significant association with stunting (p<0.001; OR=4.386) and had a direct effect on nutritional status (t=1.997; p=0.046; coef=0.130), although not influenced by maternal literacy. MGA results indicated a significant difference in the effect of growth monitoring on nutritional status between low birth weight (LBW) and normal birth weight groups (p=0.039), highlighting LBW infants as a priority for intervention. Improvements in maternal health literacy need to be made through policy advocacy at the village level, the formation of community-based support groups, and the empowerment of mothers as agents of change in society.
